You have the right to receive a copy of your health information that we maintain, with some limited exceptions.

 

Our Health Information Management Department can help you obtain a copy of your medical record. You may request access to your information in writing and also in an electronic format. You also have the right to request that your health information be sent to any person or entity.

Community's MyHealthMate is an easy, confidential way to access your medical information online and navigate our health system.
